The Motor State Diary is a daily tracking system designed specifically for Parkinson's disease management. It provides granular motor state monitoring with 30-minute precision throughout a 24-hour period.
- Tracks motor states in 30-minute intervals across a full 24-hour day
- Records five distinct motor states:
ON Normal
ON with Dyskinesia
OFF
Asleep
Not Recorded
- Organizes time into four intuitive periods: Overnight (12am-5am), Morning (6am-11am), Afternoon (12pm-5pm), and Evening (6pm-11pm)
- Features expandable/collapsible time sections for focused data entry
- Integrates with medication logging to auto-suggest motor states based on medication effectiveness ratings
- Supports detailed notes and annotations for each time slot

From the main Check-In screen, tap the Motor State button in the Quick Actions section.

3. Use the date picker at the top to navigate to any previous or future date
The diary is organized into four time periods: Overnight, Morning, Afternoon, and Evening
Tap on any period header to expand that section (only one section expands at a time)
Each period shows 30-minute intervals for precise tracking
Use horizontal scrolling to view all motor state columns
Use vertical scrolling within each expanded section
Tap on any time slot in the grid to select a motor state
Choose from five available states:
- ON Normal: Medication working well, normal movement
- ON with Dyskinesia: Medication working but causing involuntary movements
- OFF: Medication not working, symptoms present
- Asleep: Sleeping during this time period
- Not Recorded: No data for this time slot (default)
You can swipe left to find all columns. At the end of each row is where you can tap and enter notes.
1. The diary automatically shows medication times from your logged doses
2. Motor states are auto-suggested based on medication effectiveness ratings
3. Review and adjust auto-suggested states as needed for accuracy
4. Medication times appear in printed reports for healthcare provider review
1. Tap Save in the top-right corner to save your entries
2. A confirmation message appears when saving is successful
3. Tap the printer icon to generate a report
4. Printed reports include all motor states, medication times, and notes
5. Reports are formatted for healthcare provider documentation
6. Tape Done in the upper left corner of the screen when you are finished in the Diary.
Record motor states as close to the actual time as possible
Be consistent with your motor state definitions
Use notes to explain unusual patterns or events
Review your diary regularly to identify patterns
Share printed reports with your healthcare team
Complete entries for the full 24-hour period when possible
